NEW PATIENT

We would like your visit with us to be comfortable, quick and effective. You will complete any necessary paperwork upon arrival, so please arrive 15-30 minutes early to check in and complete our electronic forms before being seen at your appointment time. You will meet several members of your healthcare team. A staff member will review your chart with you and a nurse will take your vital signs. You will then go to an exam room for evaluation by one or several of our doctors.

This includes a medical history, a physical exam, orthopedic and neurological evaluation, as well as review all your medical records. If additional test are necessary, your physician will plan them with you. We have state of the art diagnostic equipment onsite – such as digital x-ray, diagnostic ultrasound, electromyography, Pro-Adjuster spinal analysis, blood testing and lab work, which may save you a trip to the hospital or numerous physician offices. Do not hesitate to ask your provider any questions you may have. We want this visit to be as thorough and comprehensive as possible which includes answering all of your questions and establishing a confidential doctor – patient relationship.
